Here are a few forms that you might need to use during your time at Heatherley:

Medicine Form - If you would like school to administer prescription medicine or pain relief to your children during the school day, please complete this form and bring it to reception with the medication.

Holiday Request / Leave of Absence - if you are taking your child out of school for a holiday (even if just for a day), please complete this request and return to reception. Please see our attendance policy and booklet for more details about holidays in term time.

Walking Home Alone (Key Stage 2 only) - Children in Year's 3 to 6 may walk home or to the gate on their own with parent consent. Please complete the form and return to reception.
