Here are a few forms that you might need to use during your time at Heatherley: 

• Medicine Form - If you would like school to administer prescription medicine or pain relief to your children during the school day, please complete this form and bring it to reception with the medication.
• Holiday Request / Leave of Absence - if you are taking your child out of school for a holiday (even if just for a day), please complete this request and return to reception. Please see our attendance policy and booklet for more details about holidays in term time. 
• Walking Home Alone (Key Stage 2 only) - Children in Year's 3 to 6 may walk home or to the gate on their own with parent consent. Please complete the form and return to reception.
